We are asking all visitors to help us look after the health and safety of our community by answering a set of standard questions about COVID-19 risk (see below).
If you answer YES to any of the questons listed below, please DO NOT VISIT our office:
In the last 14 days have you been in an area identified as high or medium risk for COVID-19? (click on the link for the current list, noting this advice can change daily)
Have you been told by Public Health to quarantine or self isolate?
Do you have cold or flu-like symptoms including fever (e.g. night sweats, chills, high temperature or new loss of taste or smell) OR symptoms of acute respiratory infection? (e.g. shortness of breath, cough, sore throat, runny nose)?
Have you had close contact with someone known to have a confirmed case of COVID-19 (either in the community OR at work) in the last 14 days?
Does anyone in your household have a fever, cold/flu-like symptoms or new loss of taste or smell, or has anyone in your household recently had a fever or temperature?
If you answered NO to all questions above and you intend to visit our office, we recommend you download Check in Tas app prior for paper and penless sign in.
